QuitWorks-NH links your patients who want to quit smoking to the full range of the state's tobacco treatment services. All it takes to use QuitWorks-NH are a clinician, a patient who uses tobacco, and the QuitWorks-NH tools.

How It Works

Using a simple referral form, any physician, nurse, or other clinician in your hospital or health center can easily and quickly enroll any patient who uses tobacco, regardless of health insurance status. The referral forms are faxed to the Try To STOP TOBACCO Resource Center of New Hampshire, the state-funded QuitWorks-NH service provider.

When the form is received, the Resource Center calls your patient, completes a patient assessment, and offers your patient the state's free, proven-effective stop-smoking services. Every referring provider will receive a fax-back report within a week to confirm contact with your patient and services accepted. About seven months later, QuitWorks-NH calls your patient to assess their smoking status and faxes a report on patient outcomes.

Who Uses QuitWorks-NH

The QuitWorks-NH program was introduced in September 2007. It is based on the original QuitWorks program developed by the Massachusetts Department of Public Health and Massachusetts health plans. It is intended for health care providers to use as a tool to aid the cessation efforts of their patients.

Since the launch of the original QuitWorks program in May of 2002, more than 1,700 office practices in Massachusetts have received a QuitWorks starter kit. Today, over 1,100 Massachusetts general practitioners and specialists alike have referred their patients to QuitWorks. Customized QuitWorks programs are in place within the Commonwealth of Massachusetts in 23 hospitals, 11 community health centers, 60 public health program sites, and more than 100 other clinics across their state.
